    • This EBP project aims to improve tracheostomy-related outcomes by developing a Standardized Tracheostomy Education Plan that comprises a discharge bundle and 10 steps for discharge. Caregivers were surveyed using the readiness for hospital discharge scale and the post-discharge coping difficulty scale with follow-up open-ended questions. Caregiver perceptions regarding their standardized trach "go-bag" were also explored and compliance was audited at every clinic visit.

Learn how color-coded labels verses handwritten labels enhanced nurse confidence and perceptions of patient safety. This session will highlight the positive outcomes from pre- and post-intervention surveys and discuss recommendations for broader application and further research.
