- This EBP project aims to improve tracheostomy-related outcomes by developing a Standardized Tracheostomy Education Plan that comprises a discharge bundle and 10 steps for discharge. Caregivers were surveyed using the readiness for hospital discharge scale and the post-discharge coping difficulty scale with follow-up open-ended questions. Caregiver perceptions regarding their standardized trach "go-bag" were also explored and compliance was audited at every clinic visit.
Outcome: 85% of the participants will self-report having gained new knowledge and/or strategies to implement into practice based on the information provided.
